Community Support/Assertive Continuing Care Model is a case management model that emphasizes working with youth and their families in the home as well as other community settings.  This modality is used as a step-down treatment for youth who have finished the Intensive Outpatient Component or as an initial treatment approach for youth with low level substance abuse symptoms.

Family Involvement is encouraged and facilitated through individual contact with parents/guardians as well as bi-weekly informational and multifamily counseling sessions.  The ultimate goal of this component is to provide families with a better understanding of the issues that are common to families of substance-abusing youth.
